Gnomon Informatics S.A.: The eHealthPass.CareCardia digital health solution for patients with chronic heart failure. Memorandum of Understanding with the 3rd Cardiology Clinic and the Faculty of Medicine at the Aristotle University of Thessaloniki (AUTH)
Approximately 200,000 people in Greece currently live with chronic heart failure, which exhibits a high mortality rate and requires systematic monitoring, simultaneously burdening the healthcare system with high annual costs.
With the aim of upgrading patient care, Gnomon Informatics S.A. offers new disease management solutions by introducing the eHealthPass.CareCardia platform—an innovative digital application for remote supervision and telehealth support for patients.
The new application is based on the internationally recognized and CE-certified medical device eHealthPass, and is already utilized in five (5) countries.
eHealthPass.CareCardia integrates artificial intelligence (AI) tools and data analytics mechanisms, ensuring continuous monitoring and personalized care tailored to each patient's needs. The use of AI enables symptom detection and the prediction of potential crises, providing supervising physicians with information for targeted interventions.
The new digital solution has been developed, under a Memorandum of Understanding, in partnership with the 3rd Cardiology Clinic of the Aristotle University of Thessaloniki (AUTH)—under the direction of Professor Vasileios Vasilikos, based at the Hippokratio General Hospital of Thessaloniki—and the Medical Physics and Digital Innovation Lab of the AUTH School of Medicine, directed by Professor Panagiotis Bamidis.
It was presented for the first time at the conference "HealthIT 2024: Innovations and Challenges in the Digital Transformation of the NHS", in Athens.
Apostolia Karampatea, Research and Development Project Manager at Gnomon Informatics S.A., and Konstantinos Bakogiannis, cardiologist and scientific associate of the 3rd Cardiology Clinic at AUTH, noted during the presentation that the new application is being piloted in Greece, Sweden, Italy, Turkey, and Portugal, serving 250 patients.
As they stated, the eHealthPass.CareCardia platform facilitates communication between patients and doctors, enabling continuous information exchange. Through user-friendly tools, patients can log data, receive notifications and reminders regarding their medication regimen, and stay informed about their treatment progress. Consequently, their quality of life is enhanced.
The core advantage of the application is the improved management of the condition—on the one hand, through effective collaboration between the patient and their attending physicians, and on the other hand, due to increased patient adherence to the treatment plan.
Ensuring data privacy and interoperability among healthcare organizations are also numbered among the platform's key features.
Furthermore, its use holds significant benefits for healthcare providers. It reduces the need for patients' physical presence in hospitals, thereby contributing to the decompression of the healthcare system and the reduction of hospitalization costs.
